Tyto skvrny se obvykle nejprve objevuj\u00ed na rukou, nohou a obli\u010deji a \u010dasto postihuj\u00ed ob\u011b strany t\u011bla symetricky.\u00a0<\/span><span style=\"font-weight: 400;\">Vitiligo postihuje mezi 0,5 a 1 % sv\u011btov\u00e9 populace a m\u016f\u017ee m\u00edt v\u00fdznamn\u00fd psychologick\u00fd a emo\u010dn\u00ed dopad (1).<\/span><span style=\"font-weight: 400;\"> Toto onemocn\u011bn\u00ed nast\u00e1v\u00e1, kdy\u017e melanocyty, bu\u0148ky produkuj\u00edc\u00ed pigment melanin, odum\u0159ou nebo p\u0159estanou spr\u00e1vn\u011b fungovat.\u00a0<\/span><span style=\"font-weight: 400;\">P\u0159esn\u00e1 p\u0159\u00ed\u010dina vitiliga zat\u00edm nen\u00ed jasn\u00e1, ale pova\u017euje se za autoimunitn\u00ed onemocn\u011bn\u00ed, kter\u00e9 m\u016f\u017ee souviset s genetick\u00fdmi zm\u011bnami a vn\u011bj\u0161\u00edmi faktory, jako je stres, sp\u00e1len\u00ed sluncem nebo expozice ur\u010dit\u00fdm chemik\u00e1li\u00edm. Pozoruhodn\u011b v\u011bdeck\u00e9 d\u016fkazy nazna\u010duj\u00ed spojen\u00ed se st\u0159evn\u00edm mikrobiomem, co\u017e nazna\u010duje, \u017ee zm\u011bny ve st\u0159evn\u00edch bakteri\u00edch mohou hr\u00e1t roli ve vzniku a progresi vitiliga.<br \/><\/span><\/p>\n<h2><strong>St\u0159evn\u00ed dysbi\u00f3za a vitiligo<\/strong><\/h2>\n<p><span style=\"font-weight: 400;\">Mnoh\u00e9 studie uk\u00e1zaly, \u017ee jedinci s vitiligem vykazuj\u00ed p\u0159edv\u00eddateln\u00e9 zm\u011bny ve st\u0159evn\u00ed mikrobiot\u011b ve srovn\u00e1n\u00ed se zdrav\u00fdmi jedinci. Zejm\u00e9na v\u00fdzkumy nazna\u010duj\u00ed, \u017ee pacienti s vitiligem mohou m\u00edt sn\u00ed\u017eenou bohatost a bakteri\u00e1ln\u00ed rozmanitost ve st\u0159evn\u00ed mikrobiot\u011b (2).<\/span><span style=\"font-weight: 400;\"> P\u0159ehledov\u00e1 studie analyzuj\u00edc\u00ed \u010dty\u0159i publikovan\u00e9 \u010dl\u00e1nky, v\u010detn\u011b t\u0159\u00ed p\u0159\u00edpadov\u00fdch studi\u00ed a jedn\u00e9 studie na my\u0161\u00edm modelu, zjistila, \u017ee jedinci s vitiligem vykazuj\u00ed st\u0159evn\u00ed dysbi\u00f3zu ve srovn\u00e1n\u00ed se zdrav\u00fdmi kontrolami (3).<\/span><span style=\"font-weight: 400;\"> Tato studie identifikovala v\u00fdznamn\u00fd pokles st\u0159evn\u00ed komenz\u00e1ln\u00ed bakterie <\/span><i><span style=\"font-weight: 400;\">Bacteroides<\/span><\/i><span style=\"font-weight: 400;\"> a zv\u00fd\u0161en\u00ed <\/span><i><span style=\"font-weight: 400;\">Pseudomonas, Psychrobacter a Butyricioccis<\/span><\/i><span style=\"font-weight: 400;\">.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Podobn\u011b studie Ni et al. (2020) uk\u00e1zala, \u017ee pacienti s vitiligem maj\u00ed odli\u0161n\u00e9 slo\u017een\u00ed st\u0159evn\u00edho mikrobiomu, kter\u00e9 se vyzna\u010duje sn\u00ed\u017een\u00fdm pom\u011brem <\/span><i><span style=\"font-weight: 400;\">Bacteroides: Firmicutes<\/span><\/i><span style=\"font-weight: 400;\"> (4).<\/span><span style=\"font-weight: 400;\"> Tento pom\u011br je d\u016fle\u017eit\u00fdm ukazatelem zdrav\u00ed st\u0159ev a jeho naru\u0161en\u00ed bylo spojeno s r\u016fzn\u00fdmi zdravotn\u00edmi probl\u00e9my. Auto\u0159i t\u00e9to studie tak\u00e9 zjistili, \u017ee ur\u010dit\u00e9 druhy bakteri\u00ed jsou u lid\u00ed s vitiligem \u010dast\u011bj\u0161\u00ed, v\u010detn\u011b <\/span><i><span style=\"font-weight: 400;\">Corynebacterium, Ruminococcus, Jeotgalibaca<\/span><\/i><span style=\"font-weight: 400;\"> a <\/span><i><span style=\"font-weight: 400;\">Psychrobacter<\/span><\/i><span style=\"font-weight: 400;\">, a \u017ee tyto bakterie v\u00fdznamn\u011b korelovaly s d\u00e9lkou trv\u00e1n\u00ed onemocn\u011bn\u00ed a hladinami proz\u00e1n\u011btliv\u00e9ho markeru IL-1\u03b2 v s\u00e9ru. Zaj\u00edmav\u00e9 je, \u017ee <\/span><i><span style=\"font-weight: 400;\">Psychrobacter<\/span><\/i><span style=\"font-weight: 400;\"> byl identifikov\u00e1n jako nejd\u016fle\u017eit\u011bj\u0161\u00ed prediktor vitiliga. To bylo doprov\u00e1zeno zv\u00fd\u0161enou aktivitou gen\u016f spojen\u00fdch s bakteriemi, kter\u00e9 mohou naru\u0161it ochrannou st\u0159evn\u00ed sliznici. Mezit\u00edm jin\u00e1 studie pouk\u00e1zala na v\u00fdznamn\u00e9 zm\u011bny v metabolick\u00fdch drah\u00e1ch pacient\u016f s vitiligem (6).<\/span><span style=\"font-weight: 400;\"> Konkr\u00e9tn\u011b bylo zji\u0161t\u011bno sn\u00ed\u017een\u00ed degradace cysteinu, co\u017e by mohlo potenci\u00e1ln\u011b v\u00e9st ke sn\u00ed\u017een\u00ed hladiny antioxidant\u016f, a zv\u00fd\u0161en\u00ed degradace galakt\u00f3zy.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Ned\u00e1vn\u00e1 systematick\u00e1 p\u0159ehledov\u00e1 studie publikovan\u00e1 v lednu 2025 shrnuje aktu\u00e1ln\u00ed v\u00fdzkumn\u00e9 poznatky o roli st\u0159evn\u00edho mikrobiomu u vitiliga (7).<\/span><span style=\"font-weight: 400;\"> Auto\u0159i t\u00e9to studie uzn\u00e1vaj\u00ed nekonzistentn\u00ed v\u00fdsledky mezi \u0161esti zahrnut\u00fdmi studiemi kv\u016fli rozd\u00edl\u016fm v metodologi\u00edch, velikostech vzork\u016f a vn\u011bj\u0161\u00edch faktorech, jako je strava a \u017eivotn\u00ed styl.
